It affects millions of people worldwide and is highly prevalent among children and women. Epidemiological studies have identified herpes simplex virus type 1 and the mammalian epidermal growth factor receptor as etiological factors, while there is evidence for genetic susceptibility factors. However, the pathogenesis of the disease is still unknown. AA is generally considered to be an autoimmune disease of the skin in which genetic susceptibility and environmental factors are important determinants of disease expression. Cytokines, proteins produced by the immune system, can modulate the immune response to foreign antigens, and abnormalities in cytokine production have been linked with many immune disorders. The aim of the present study was to evaluate the occurrence of AA in patients with various immune disorders (rheumatoid arthritis, psoriasis, systemic lupus erythematosus, and myasthenia gravis), to compare the serum cytokine levels in AA and normal control groups, and to examine whether the serum levels of cytokines would be affected by immunotherapy. Twelve patients with moderate AA were enrolled. Serum levels of tumour necrosis factor alpha (TNF-alpha), interferon gamma (IFN-gamma), interleukin 10 (IL-10), and transforming growth factor beta (TGF-beta) were determined using enzyme-linked immunosorbent assay (ELISA). Serum levels of TNF-alpha and TGF-beta were higher in the AA group compared with the control group (P < 0.05). The levels of IL-10 and IFN-gamma did not differ significantly between the two groups. There were no correlations between TNF-alpha, TGF-beta, IFN-gamma, IL-10, TNF-alpha, and TGF-beta levels and clinical parameters of AA. IL-10 and IFN-gamma were lower in the clinical response group than in the non-response group (P < 0.05). Serum TNF-alpha, TGF-beta, IFN-gamma, and IL-10 levels in AA were increased compared with controls and did not correlate with clinical and laboratory findings.
